I requested premium, attendant pumped a lower grade. Pump did not display accurate information until after fuel was dispensed (all zeros during filling). Attendant failed to screw the cap onto the gas tank. I noticed the open fuel door after I left, and when I went to close it saw the cap just hanging there. Will not use a
